Ah, when reinstalling P3dv4.1 it says it is installed but the Prepare3D.exe is 4.0!! Strange. May have to do a complete reinstall again.

On startup the usual small P3d black screen appears for some seconds then disappears. The task manager shows it running with the CPU at 15% but there is no screen to access. Removing FSUIPC5 allows P3d to run normally. The Events log shows nothing. It is newly registered. FSUIPC5.ini is empty.
