FSUIPC4 Issues

Hello Pete: I'm a newbie and am having trouble with the FSUIP4. I don't get a FS Toolbar Menu upon keying Alt+Enter, thus no FSUIPC functionality. I've attached log and other pertinent files for your review. IMHO all appears OK. I've been reviewing all the posts for fixes but none seem to work. Your help would be welcomed.
